Addressing the Social Determinants of Health
Health and wellness is so much more than going to the doctor or getting prescriptions filled. If individuals and families leave the physician’s office and return home to environments that don’t promote or sustain healthy behaviors, treating diseases like asthma, depression or heart disease can become difficult or even impossible. The social determinants of health influence everything else in our health care system.
Wheeler Recognized for Patient-Centered Care, Quality
The only community health center in Connecticut with both primary medical care and behavioral health home certifications, Wheeler is also one of five organizations nationally to earn four specific accreditations and certifications from the nation’s oldest and largest standards-setting and accrediting body for providing quality, coordinated, patient-centered health care.
Expanded Statewide Support for Youth in Recovery
Wheeler has launched a new statewide program to provide additional support to youth in recovery. The Connecticut Recovery-Oriented Support System for Youth (CROSS) initiative is funded by the Substance Abuse and Mental Health Services Administration (SAMHSA) 2017 State Targeted Response to the Opioid Crisis grant (STR) through the Connecticut Departments of Mental Health and Addiction Services (DMHAS) and Children and Families (DCF).
Promoting Recovery When Transitioning Home
A new program supports adults recovering from a substance use disorder as they transition back to their home and community. The Wheeler/Anthem Recovery Care Facilitation program supports adults who are insured by Anthem Blue Cross and Blue Shield of Connecticut and facilitates their transition from a hospital, or residential facility to the community or home.
