We want to make sure that everyone we serve gets the best care possible. Help us get to know you better!  The registration form received upon your first appointment, will ask you some detailed questions regarding your (or your child’s) race and ethnicity, sexual orientation and gender identity.  This helps ensure that we give the best experience and highest quality of care to everyone. 

What do the terms race, ethnicity, sexual orientation and gender identity mean?

• “Race” refers to the group or groups that you identify with as having similar physical characteristics (skin color, hair type, eye shape) or similar social and geographic origins. 
• “Ethnicity” is a term which represents social groups with a shared culture, which includes shared history, sense of identity, traditions, geography, and cultural roots.
• “Sexual Orientation” is about who a person is physically/emotionally attracted to. 
• “Gender Identity” is a person’s internal sense of their own gender, whether that is man, woman, neither, both, or something else.  One’s gender identity may or may not correspond to their assigned sex.

We will keep your information confidential and will update it in your record. 
We are also going to ask you about the language(s) that you speak. If you prefer a language other than English, we will find someone to speak to you in your preferred language and provide translated written materials.

Q: WHAT DOES MY RACE, ETHNICITY, SEXUAL ORIENTATION AND GENDER IDENTITY HAVE TO DO WITH MY HEALTH AND WELLBEING? 
A: Although we are all individual people, studies show that our unique backgrounds may place us at differing risks for some diseases or other disparate social or educational outcomes and experiences. We can work to reduce these risks by making sure that everyone gets high quality care and equitable outcomes regardless of their background. 

Q: WHO ARE YOU COLLECTING THIS INFORMATION FROM? 
A: We are collecting this information from everyone that we serve. 

Q: WHAT WILL MY INFORMATION BE USED FOR? 
A: By sharing information about yourself, you are helping us provide better services and programs to you and to others like you. For example, with this information, we can provide information in languages spoken by the individuals that we serve and offer effective programs and services that can improve everyone’s health and wellbeing. 

Q: WHAT IF I DON’T WANT TO ANSWER THESE QUESTIONS? 
A: It is perfectly alright if you do not want to answer some or all of the questions. However, this information helps us provide better care and services to you.  Whether or not you answer these questions, we will still provide you the very best possible care and services. 

Q: WHO WILL SEE MY INFORMATION? 
A: The only people who will see your information are members of your care/service team and others who are authorized to see your record. 

Q: WHAT IF I DON’T KNOW MY RACE, ETHNICITY, SEXUAL ORIENTATION OR GENDER IDENTITY? 
A: You can answer the questions by selecting a response closest to how you identify yourself.  You can also select “declined to specify” if you prefer not to answer any question, or you can say you do not know if you are unsure.  If you need, our staff can help you decide the best way to answer.

¿Qué significan los términos raza, etnicidad, orientación sexual e identidad de género?

• “Raza” refiere al grupo o grupos con los cuales usted se identifica con respeto a las características físicas parecidas (color de piel, tipo de pelo, forma del ojo) u orígenes sociales y geográficos similares.
• “Etnicidad” es un término que representa los grupos sociales dentro de una cultura compartida, los cuales incluyen historia, sentido de identidad, tradiciones, geografía, y raíces culturales compartidas.
• “Orientación sexual” se trata de a quién una persona se siente atraída física o emocionalmente.
• “Identidad de género” es el sentido interno de una persona sobre su propio género, sea que es un hombre, mujer, ninguno, ambos, u otra cosa. Puede que la identidad de género de uno corresponde a su género asignado o puede que no corresponda.

Mantendremos su información confidencial y actualizaremos su informe. 
También le vamos a preguntar sobre el(los) idioma(s) que habla. Si usted prefiere un idioma que no es el inglés, buscaremos a alguien que le puede hablar en su idioma de preferencia y proveerle traducciones de los materiales escritos.

R: Aunque todos somos personas individuales, las investigaciones demuestran que nuestras historias únicas nos pueden arrojar riesgos diferentes para algunas enfermedades u otro resultado o experiencia social o educación diferente. Podemos tratar de reducir estos riesgos a través de asegurar que todos reciban atención de alta calidad y resultados equitativos no importa su historia. 

P: ¿DE QUIÉN ESTÁ RECOPILANDO ESTA INFORMACIÓN?
R: Recopilamos esta información de todos a quienes servimos.

R: Mediante compartir información sobre usted mismo/a, nos está ayudando a proveer mejores servicios y programas para usted y otros como usted. Por ejemplo, con esta información, podemos proveer información en los idiomas que hablan los individuos quienes reciben nuestros servicios y ofrecer programas y servicios eficaces para mejorar la salud y el bienestar de todos.

R: No hay ningún problema si no quiere responder a algunas o ninguna de las preguntas. Sin embargo, esta información nos ayudar a proveer mejor atención y servicios para usted.  De todos modos, si contesta estas preguntas o no, le proveeremos el mejor cuidado y servicio posible.

R: Las únicas personas quienes verán su información son los miembros del equipo de servicio/atención y otros quienes sean autorizados para ver su informe.
