The Path to Sobriety is Hard. Wheeler Can Help.

 For teens and young adults, finding the path to a healthy, happy life can be challenging, even amid the most optimal circumstances. For young people with struggling substance use or other life challenges—particularly as they are transitioning to adulthood—finding that path can prove to be even more difficult. Wheeler’s Youth and Family TREE program can help by supporting teens and young adults, ages 16 to 25, and their families, as well as community providers who care for them. Our goal is to assist teens and young adults in getting connected to treatment and linking them with needed resources in order to stay connected to treatment.

PROVIDERS

TEENS/YOUNG ADULTS

 Our caring staff work with transition-age youth to identify substance misuse or related risks for that youth and their family members, and we provide recovery supports and education to reduce risk. We also work with youth and families to determine other unmet needs such as transportation, shelter, food, clothing, medical or child care, or insurance and financial assistance.

We provide:

  • Education for teens and young adults about the dangers of substance misuse and resources for prevention, intervention and recovery support.
  • Substance Misuse and Risk Screening for all 16-25 year olds seen at 43 Woodland Street, regardless of primary services being received (medical or behavioral health).
  • Behavioral health outpatient services designed to meet the needs of teens and young adults.
  • Connection to recovery supports and resources to promote well-being and support for family members, including parents, partners, siblings, and children to  to identify their needs to help them assist the young person’s sustained recovery.
  • SBIRT for family members of program participants

PROVIDERS                                                                            

Your role is as a behavioral health provider is challenging, with many details to manage. We understand that it’s often hard to keep your teens and young adult patients focused. Wheeler’s Youth and Family TREE program can support you.

Our caring team works collaboratively with you to identify the issues and needs of your clients that might interfere with their participation in treatment, which ultimately can affect their short- and long-term outcome in care. Our team closely works with you to understand your clients’ needs and the needs of their families, and we identify and provide linkages to other forms of care, including medical and behavioral health care through our community health centers, and other community support services. We also work with youth and families to determine other unmet needs such as transportation, shelter, food, clothing, childcare, or insurance and financial assistance to help young adults thrive.

This evidence-based program works. Our staff collaborate closely with clients to help them navigate systems of care, stay organized, manage the details of staying connected to treatment. We guide individuals through treatment, and we help them manage the details required to build and maintain a productive daily living routine, from setting appointments and sending reminders, to helping to secure bus passes and schedules, or other resources.

We provide:
