Transitional Case Management

favicon_144x144.pngWheeler’s Transitional Case Management (TCM) program supports adult men transitioning from incarceration into the community. We provide comprehensive case management and relapse prevention/substance use services. Each individual in TCM must have a substance use diagnosis. We typically work with men three to four months prior to the release from incarceration, and an additional 90 to 120 days while participants are in the community. We offer temporary structure and support to help people succeed while they are navigating the community following incarceration. Please note: This is not a sponsorship program.


The program* serves males 18+:

  • With a substance use diagnosis who have been released from the Department of Correction, including individuals with Probation, Parole or leaving a halfway house.
  • Without acute mental health needs requiring intensive outpatient services.
  • Who are able to engage in TCM Group for case management and relapse prevention.
  • Who are in remission from substance use, and who are either in the “action” or “maintenance” stage of change.
  • Who are willing to engage in self-help groups, including AA/NA/CCAR/SMART RECOVERY

*Individuals may be referred by the State of Connecticut Judicial Branch, Court Support Services Division.


The program provides:

  • Creation of a strengths-based transition plan to meet specific needs.
  • Linkage to support services such as employment, sober housing, medical insurance, transportation, and other resources.
  • Continued weekly meetings with members of the TCM team.
  • The TCM clinician and case manager work closely with individuals during their transition period, providing:
  • Encouragement and support
  • Mentoring and guidance
  • Education
  • Coordination of services
  • Advocacy
  • Counseling and crisis intervention
  • Monitoring and follow up

The program supports men who intend to live in New Britain, Plainville, Bristol, Middletown, Meriden or Greater Hartford.


The program is funded by the Connecticut Department of Mental Health and Addiction Services.
