The Southington Health Outreach Project

Wheeler’s Southington Health Outreach facilitates access to behavioral and mental health care for Southington residents. The project employs a three-tiered approach to serving individuals and families in this community, including outreach and health education; system navigation assistance; and efforts to reduce barriers to care such as transportation, child care concerns and others.

A full-time senior community health outreach coordinator collaborates with key community organizations to reach school-aged, low-income, elderly and other vulnerable populations and provide direct assistance to individuals in scheduling appointments; linkage with certified professionals who can assist with Medicaid and Medicare enrollment; and connection to substance abuse treatment and recovery, health care services, early childhood and other programs and services.

Since its inception, the Southington Health Outreach Project has reached hundreds of Southington residents through the Southington Public Library, Calendar House Senior Center, Southington Chamber of Commerce, Southington Housing Authority, Southington Community YMCA, civic and faith groups, grocery stores, pharmacies, and numerous other community organizations. The project also has connected many residents to services, including primary and behavioral health care, and linkages with other financial and disability services.

The Southington Health Outreach Project is funded by the Bradley Henry Barnes and Leila Upson Barnes Memorial Trust at the Main Street Community Foundation.
