The Individualized Family Service Team offers a home-based network of
resources for families and their children at risk for out of home placement.
The program integrates community supports and intensive case management
services to help a child remain successfully in the home environment.
The program utilizes a flexible, multi-disciplinary approach to treatment
that involves the whole family. The team develops a comprehensive intervention
plan that encompasses a clear set of goals, objectives and community resources
to meet the unique needs of the child and family. Interventions are based
on a comprehensive inventory of the family's strengths and identification
of crucial areas needing change to preserve an environment that is safe
and nurturing.
