Making the important transition from youth to adulthood is difficult for any teen, but especially one who has been in foster care or other types of temporary placement, or has few family connections. Life Long Family Ties (LLFT) is an innovative program that works to create a network of caring adults to help guide a youth's transition into young adulthood and beyond. In addition to the coordination of services, our team of experts helps ensure an optimal degree of safety, support and permanency. Foster care and residential placement is also available when it is needed.
Life Long Family Ties is available to youth who are:
- 13 to 18 years old
- Referred by the Department of Children and Families
- Currently in foster care or residential programs
- Have a future goal of permanent foster care or independent living
